At our practice, we take pride in treating our patients with respect and special care. We know that dental treatment can be anxiety-inducing. We believe in taking time to treat our patients like cherished guests.
From the moment you enter our practice, you will be greeted by team members who are devoted to making your visit a pleasant one.
Your first visit will include a thorough examination of your dental condition and specific periodontal needs. We will complete an in-depth medical and dental health history and a comprehensive periodontal examination to measure for periodontal attachment and bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease. We will review your concerns and any special preferences you may have about treatment. We will review your periodontal condition and any treatment recommendations in detail.
We know your time is valuable. We are dedicated to being punctual and efficient.
Please assist us at the time of your initial visit to the office by providing the following information:
- Your referral documentation
- A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website)
- If you have dental insurance, please bring any forms or insurance cards with you to the appointment
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).
